Admissions

Admission into the nursing home is easily started by calling and speaking with our Admissions Coordinator.  You are encouraged to tour the facility and sit down with our staff to discuss your situation.

Medical records will be reviewed for potential needs and to determine appropriateness for admission.

For admissions into skilled nursing following a qualified hospital stay, the hospital discharge planner or case manager can handle the referral and send the information needed to TLC Nursing Center.

During the initial process, various funding options can be reviewed to help you decide what is best for you or your loved one.
